Remote UKallergy: ALEX³ Test Order Form, Symptom Questionnaire and Card Payment

To help our consultants interpret your results accurately, we ask that you describe your own experience rather than referring to previous test results or diagnoses.     Once payment has been made via our secure Worldpay system, your testing kit will be dispatched to your home address. You will receive instructions explaining how to collect your sample using a finger-prick method and post it to the laboratory. If you are not comfortable collecting your own sample, please do not proceed with payment. Instead, contact our office at info@ukallergy.com , and we will be happy to discuss alternative arrangements with you. For children, samples must be collected by a healthcare professional.
